29 CFR 1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift. At the worksite located near 3600 Blue Ridge Road in Grandview Missouri 64030: Employees performing tree trimming work were exposed to falls greater than 10 feet above the ground while working inside the bucket of an aerial lift without wearing any means of fall protection or a personnel fall arrest system.

29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not de-energized and grounded, nor were other protective measures provided before work was started: The work practices used by qualified persons installing insulating devices on overhead power transmission or distribution lines are not covered by 29 CFR 1910.332 through 29 CFR 1910. 335. Under 29 CFR 1910.333(c) (2) unqualified persons are prohibited from performing this type of work. At the worksite located near 3600 Blue Ridge Road in Grandview Missouri 64030: Employees performing tree trimming work were exposed to shock, burns, and electrocutions, from working near power lines that were not de-energized and grounded. The employees were working inside the basket of an aerial lift next to energized power lines, without any insulation around them.
