On or about January 11, 2023, at the address of 33460 Lake Rd. Avon Lake, OH 44012. The employer did not ensure employee(s) were protected from fall hazards while working atop a low sloped commercial structure. Employee applying Karnak 19 flashing cement atop the low sloped commercial roof structure was exposed to a fall of 24 feet to the ground below.
