29 CFR  1928.57(d)(1)(v): Chains and sprockets were not completely enclosed:  On or about January 7, 2023, employees engaged in cotton gin activities at the module feeder were exposed to being caught in chain and sprockets not fully enclosed.  Instances include, but are not limited to:  a) Located under the catwalk, on the right side coming from the entrance of the unloading area on the floor, the sprocket and chain system to the motor was not guarded.  b) Roller conveyor drive sprocket and chain system of the Feeder Module was missing the expanded metal enclosure.

29 CFR  1928.57(d)(3): A warning device was not  installed in the gin to provide an audible signal which will indicate to employees that any or all of the machines comprising the gin are about to be started. The signal was not of sufficient volume to be heard by employees and sounded each time before starting the gin:   a) On or about January 7, 2023, employees performing activities such as, but not limited to cleaning on the roller conveyor were exposed to caught-in hazards when the audible alarm in place for the roller conveyor did not provide enough warning prior to and before the equipment started.

29 CFR  1928.57(a)(6)(iii):  Instruction was not provided, to include the steps and procedures necessary,  to safely service or maintain equipment by stopping engines, disconnect the power sources, and waiting for all machine movement to stop before servicing, adjusting, cleaning, or unclogging the equipment:  a) On or about January 7, 2023, employees engaged in cleaning activities at the module feeder were not instructed on the procedures and steps necessary to ensure all engines are stopped, power disconnected and the machine movement had stopped prior to performing cleaning operations.
