29 CFR  1926.251(c)(5)(i):The U section of U-bolt wire rope clip(s) used to form eyes was not in contact with the dead end of the rope:  (a) 600 Penn Street, Huntingdon PA, Blair House Apartments demolition site - On or about January 20, 2023 the employer used two (2) joined 7/8 inch steel cables while conducting demolition which had at least two 1 1/4 inch Crosby Clips which did not have the U section of the U-bolt in contact with the dead end of the rope.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   (a) 600 Penn Street, Huntingdon PA, Blair House Apartments demolition site - On or about January 20, 2023 the employer  did not ensure employees were trained and  understood the torqueing requirements need for the four 1 1/4 inch Crosby Clips.  (b) 600 Penn Street, Huntingdon PA, Blair House Apartments demolition site - On or about January 20, 2023 the employer  did not ensure employees were trained and  understood the proper way to install Crosby Clips on a wire.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
