29 CFR 1926.502(b)(2):Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not installed between the top edge of the guardrail system and the walking/working surface when there is no wall or parapet wall at least 21 inches (53 cm) high: 20 Lamartine St., Worcester, MA: On or about October 31,2022, an employee receiving wall board through an opening on the 4th floor was exposed to fall hazards due to the lack of a midrail at this opening.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: The guardrail failed when the employee leaned on it. 20 Lamartine St., Worcester, MA: On or about October 31,2022, an employee receiving wall board through a wall opening on the 4th floor leaned on the installed guardrail which failed when the employee leaned on it and the employee fell more than 50 feet to a dumpster located on the ground below the opening. The guardrail system installed in the opening was not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge.
