The entry did not include the object or substance which caused the injury.  b) Facility - On or about January 10, 2023, the employer did not complete the OSHA Form 300 for calendar year 2023 in the detail required in that an injury, which occurred on January 26, 2023 was described as "Hand Shoulder and Shoulder Joint-Right part-Sprain-Ergo, Shoulder and Shoulder Joint-Right" in column F and "Lab" in Column E. The entry did not include the object or substance which caused the injury.   e) Facility - On or about January 10, 2023, the employer did not complete the OSHA Form 300 for calendar year 2023 in the detail required in that an injury, which occurred on March 8, 2023 was described as "Hand Hand-Left part-Sprain-Ergo, Hand-Left part-Ergo, repetitive Motion, Hand and thumb pain from various material" in column F.
