The employer requires spray-painters employees to wear 3M half-face/ dual filter respirators filtering respirators when spray painting without having been provided medical evaluations prior to respirator use.

In the back storage room, employee(s) engaged in forklift production activities were exposed to struck by hazards in that no forklift training program existed where forklift operators could be trained for the safe operation of forklifts.
