29 CFR  1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  In the back storage area, employee(s) engaged in production activities were exposed to fire and explosion or a projectile hazards in that compressed gas cylinders, that were not in use, were not secured to prevent damage in accordance with Compressed Gas Association Pamphlet P-1-1965 paragraph 3.3.8.

29 CFR  1910.134(e)(1):The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:(a)   In the finishing area, employees are exposed to pulmonary hazards and/or cardiac stress hazards when using employer required respirators without having been provided a medical evaluation prior to use. The employer requires spray-painters employees to wear 3M half-face/ dual filter respirators filtering respirators when spray painting without having been provided medical evaluations prior to respirator use.

29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  In the back storage room, employee(s) engaged in forklift production activities were exposed to struck by hazards in that no forklift training program existed where forklift operators could be trained for the safe operation of forklifts.
