On or about January 3, 2023 at location 23 Kilbourn Street Kewaunee, WI: employees using safety pull back devices to keep their hands out of the point of operation when operating the Bliss 3659 full revolution power press were not protected due to the pull back devices were not adjusted correctly and the operator was able to have their hands in the point of operation while the die was closing.
