29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: a) Jobsite, 1301 Paulson Avenue, Pittsburgh, PA 15206: On or about November 25th, 2022, employees working in close proximity to overhead power lines were exposed to the hazards of electric shock and electrocution. An employee received serious injuries after materials he was handling contacted overhead power lines (13.2 kV phase to ground).

29 CFR 1926.416(a)(3): When and where energized circuits existed, the employer failed to post and maintain proper warning signs, advise employees of the location, hazard involved and the proper protective measures to be taken: a) Jobsite, 1301 Paulson Avenue, Pittsburgh, PA 15206: On or about November 25th, 2022, overhead power lines (13.2 kV phase to ground) were located in close proximity to employees working on the roof of the house. The employer did not post and maintain proper warning signs, advise employees of the location, hazard involved and the proper protective measures to be taken. An employee received serious injuries when when materials he was handling contacted the overhead power lines.
