29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  Firearm Invoice Storage Cage:  On and/or before January 12, 2023, employees accessed and walked the top of the Firearms Vault which had an unprotected edge and was above 4 feet high from the next lower level to retrieve firearms documentation from storage.  The employees did not use fall protection systems while accessing and moving in the area which potentially exposed the employees to approximately 18 foot fall hazards.

29 CFR  1910.23(c)(11):  The employer did not ensure that portable ladders used to gain access to an upper landing surface that have side rails that extend at least 3 feet (0.9 m) above the upper landing surface.  Firearm Invoice Storage Cage: On and/or before January 12, 2023, employees used fiberglass A-frame ladders to access the top of the Firearms Vault which had an unprotected edge and was above 4 feet from the next lower level to retrieve firearms documentation.  The employer did not ensure ladders used extended at least 3 feet above the upper landing surface which potentially exposed the employees to approximately 18 foot fall hazards.

Firearm Invoice Storage Cage:  On and/or before January 12, 2023, employees used fiberglass A-frame ladder(s) top steps to access the top of the Firearms Vault which had an unprotected edge and was above 4 feet high from next lower level to retrieve firearms documentation.  The employer did not ensure the top steps on the ladders were prohibited from use in accordance with the ladder design which potentially exposed the employees to approximately 18 foot fall hazards.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  Administrative Bull Pen: Sign Area:  On and/or before January 12, 2023, employees used a portable extension cord(s) as a source for permanent power to power items such as a Hewlett Packard Color Laser Jet M653 copier/printer to create advertisement signs used in the retail store.  This condition potentially exposed employee(s) to electrical shock and/or fire hazard(s).

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Reference:  ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.6 The eyewash unit temperature delivered by emergency equipment should be 'tepid' (between 60�F (16�C) and 100�F(38�C)) which involves providing both hot and cold water to the unit and installing a mixing valve to blend the water to the desired temperature.  Food Prep Station: On and/or before January 12, 2023, employees used corrosive cleaners such as Sysco Classic Germicidal Ultra Bleach to clean food preparation sink area. The emergency eye wash unit was designed to where the delivery of 'tepid' water could not be consistently achieved which potentially exposed the employees to retinal or corneal burns.
