1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and were not done in the following sequence: a. True Label Inc., 946 Kane Street suite B, Toledo, Ohio 43612: On or about January 5, 2023, the employer did not ensure the point of operation on each of the eleven printing stations on the Aquaflex LCOX, Press 1 were adequately guarded. Employees wiping the cylinder printing plate during production were exposed to caught in hazards between the cylinder and the anilox roller.
