Abarta Coca-Cola Beverages, Warehouse, on or about 1/13/23 and times prior thereto: procedures for control of hazardous energy were not developed, documented, and utilized by employees performing servicing and maintenance tasks on trucks, tractors, and trailers. b.)Abarta Coca-Cola Beverages, Warehouse, on or about 1/13/23 and times prior thereto: procedures for control of hazardous energy were not developed, documented, and utilized by employees performing servicing and maintenance tasks on powered industrial trucks. c.)Abarta Coca-Cola Beverages, Warehouse, on or about 1/13/23 and times prior thereto: procedures to ensure exclusive control and access to lockout devices were not developed, documented, and utilized. Employees at times did not maintain control over lockout devices by putting keys in tool boxes. Employees perform servicing and maintenance tasks on trucks, tractors, trailers, and powered industrial trucks. Employees perform service and maintenance tasks on equipment including, but not limited to fork trucks, tractors, trucks, and trailers. No procedure was created, developed, or implemented to provide protection to the employees that would be equivalent to the protection provided by a personal lockout device. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. The employer had not received prior written permission from the forklift manufacturer to use the attachment. b.) Abarta Coca-Cola Beverages, Warehouse, on or about 1/13/23 and times prior thereto, a lifting and dumping attachment was being used with equipment including, but not limited to, CAT Model #E6500-AC Serial # A4EC 420287. The attachment was used for work operations including, but not limited to, dumping trash cans into dumpsters. The employer had not received prior written permission from the forklift manufacturer to use the attachment. C.) Abarta Coca-Cola Beverages, Warehouse, on or about 1/13/23 and times prior thereto, a Self-Dumping Steel Hopper was being used with equipment including, but not limited to, CAT Model #E6500-AC Serial # A4EC 420287.
