29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  (a) On or about January 13, 2022, employees that conduct blade changes and other servicing on saws, pallet machines, and other equipment, were not provided with a hazardous energy control program that included training, procedures, and periodic inspections.

29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) On or about January 13, 2023, employees that conduct blade changes and other servicing on the pallet machine, were not provided with hazardous energy control procedures that describe the hazardous energy sources (such as, but not limited to; electrical and hydraulic), and the means/methods to isolate and control those hazardous energy sources.

29 CFR  1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  On or about January 13, 2023, chain and sprockets on machinery were not guarded to prevent employees from accessing them while the machinery was in operation, in that:  (a) The sprocket and chain assembly feeding into the splicer was not guarded, and;  (b) The chains and sprockets for the conveying system immediate after the pallet machine assembly was not guarded.
