29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about January 10, 2023, employees were conducting tear-off and roof replacement of the center tower antenna roof without the use of a fall protection system. This exposed employees to a twelve-foot fall hazard onto the lower main roof of the facility.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines: (a) On or about January 10, 2023, employees accessed the roof from an extension ladder to access the antenna tower roof to perform work. The area from the ladder landing to the antenna roof was not connected by an access path formed by two warning lines.
