29 CFR 1926.251(a)(2)(iii): The employer did not ensure that rigging equipment not be used without affixed, legible identification markings required by paragraph (a)(2)(I) of this section. On or about January 10, 2023, employees rigged materials using a four leg chain spreader that had not been affixed with legible identification markings, exposing employees to working around unsafe work load.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement. On or about January 10, 2023, and at times prior thereto, employees were connecting chains to columns and installing wallers onto columns while working above rebar that was not guarded, exposing employees to impalement hazards.

29 CFR 1926.1428(c)(5): Each signal person did not demonstrate that he/she meet the requirements in paragraphs (c)(1) through (4) of this section through an oral or written test, and through a practical test.
