1910.145(c)(3) Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): a) Structural Department (OBRF/Flange Area): On or about January 10, 2023, and at times prior, the employer exposed employees to amputation and caught-in hazards, in that adequate instruction signage for the infeed roller assembly was not provided to employees while operating the Flange Line machine. b) Structural Department (OBRF/Splice Plate Area): On or about January 10, 2023, and at times prior, the employer exposed employees to amputation and caught-in hazards, in that adequate instruction signage for the "Y" Unit assembly was not provided to employees while operating the Splice Plate Line machine.
