29 CFR  1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, was not worn and attached to the boom or basket when working from an aerial lift.  (a) 2833 Kellysville Road, Princeton, WV: On January 11, 2023 an employee was working from an aerial lift assisting in the lashing of a new fiber optics line. The employee was not utilizing a personal fall arrest or travel restraint system, exposing them to a fall hazard.

(a) 2833 Kellysville Road, Princeton, WV: On January 11, 2023 an employee was working in proximity to overhead primary powerlines from an aerial lift, assisting in the lashing of a new fiber optics line. The employer did not ensure the employee utilized head protection, exposing them to possible high voltage electrical contact.

29 CFR  1910.268(b)(7): The employer did not ensure that no employee approached or took any conductive object closer to any electrically energized overhead power lines and parts than prescribed in Table R-2 or in (i) - (iii) of this section:   (a) 2833 Kellysville Road, Princeton, WV: On January 11, 2023 an employee was working in proximity to a 19,000 volt overhead power line from an aerial lift, assisting in the lashing of a new fiber optics line. The employee operated the equipment such that their body was within 36 inches of the electrically energized power line.
