(a) 2833 Kellysville Road, Princeton, WV: On January 11, 2023 an employee was working from an aerial lift assisting in the lashing of a new fiber optics line. The employee was not utilizing a personal fall arrest or travel restraint system, exposing them to a fall hazard.

The employer did not ensure the employee utilized head protection, exposing them to possible high voltage electrical contact.

29 CFR 1910.268(b)(7): The employer did not ensure that no employee approached or took any conductive object closer to any electrically energized overhead power lines and parts than prescribed in Table R-2 or in (i) - (iii) of this section: (a) 2833 Kellysville Road, Princeton, WV: On January 11, 2023 an employee was working in proximity to a 19,000 volt overhead power line from an aerial lift, assisting in the lashing of a new fiber optics line. The employee operated the equipment such that their body was within 36 inches of the electrically energized power line.
