29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a. Northeast Corner of the building: One 120 volt flexible cord hanging from the ceiling near Carrier Case RR1 was daisy chained between a mix of relocatable power taps, extension cords and carrier lights so that this one power cord provided lighting and power for Carrier Cases RR1; RR13, RR12, RR10 and RU01. This violated the listing that relocatable power taps are not be daisy chained to other relocatable power taps or plugged into extension cords. ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) Center of production area: No visual inspection had been conducted on any shift on the Southworth Products Corp Portable Container Tilter due to a missing ground prong and a damaged cord which exposed insulated wires underneath. Instead of having it fixed someone had tied rubber gloves over the break in the insulation. ABATEMENT CERTIFICATION IS REQUIRED
