29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: a) Worksite: The area near a 31-inch in diameter outside manhole located between the job trailer and the corner of the building was littered with PVC pipe, utility tubing, commo and electrical cords and cables, hoses, scrap lumber and concrete debris, creating tripping hazards for employees.

29 CFR 1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing safety and heath hazards and conditions) a) Worksite: The employer failed to have the worksite frequently inspected by a competent person who could identify and correct safety and health hazards . This is reflected by a uncovered manhole opening and the ground nearby littered with debris and work materials such as cable for over a month unabated. ABATEMENT CERTIFICATION IS REQUIRED
