29 CFR 1910.178(l)(4)(ii)(B):29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics shall be provided to the operator when: The operator has been involved in an accident or near-miss; On or about January 6, 2023 the employer did not ensure that a powered industrial vehicle (PIV) operator had received an evaluation and refresher training prior to operating a PiV again. An employee was able to finish a shift which included fork truck operation following a documented accident.
