29 CFR 1926.452(i)(8):  Scaffolds and scaffold components were not designed by a registered professional engineer and were not constructed and loaded in accordance with such design:   Stony Brook University Hospital: 101 Nicolls Rd, Stonybrook, NY 11794  a) Worksite: Main Building Roof: On or about January 17, 2023, employees were exposed to struck-by/ crush-by hazards, while setting up a temporary outrigger scaffolding system that collapsed while being constructed.  Employees did not construct the system as it was designed by the registered professional engineer.  Note: Abatement documentation is required pursuant to 29 CFR 1903.19.
