29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.   a) On or about January 10, 2023, employees in the cut shop operating the SCM Olimpic K230 Evo edge bander were exposed to rotating/moving parts and unguarded end trimming unit saw blades due to open soundproof covers, thereby exposing employees to flying debris and caught-in hazards.

29 CFR  1910.147(c)(4)(i):Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.  a) On or about January 10, 2023, employees operating the SCM Olimpic K230 Evo edge bander failed to utilize control of hazardous energy procedures while engaged in servicing and/or maintenance activities, such as but not limited to re/filling glue, loading edge banding, unjamming and clearing debris, thereby exposing employees to caught-in and/or struck-by hazards.   b) On or about January 18, 2023, employees operating the Whirlwind cut-off saw failed to utilize control of hazardous energy procedures while engaged in servicing and/or maintenance activities, such as but not blade changes, thereby exposing employees to caught-in and/or struck-by hazards.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  a) On or about January 10, 2023, employees were exposed to machine hazards associated with moving parts, pinch points, and unguarded saw blades during unjamming and clearing debris on the SCM Olimpic K230 Evo edge bander, located in the cut shop. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  b) On or about January 18, 2023, employees were exposed to machine hazards associated with moving parts and an unguarded saw blade during a blade change on the Whirlwind cut-off saw, located in the cut shop. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
