29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (a) On or about December 13, 2022 and thereafter, at an establishment located at 501 Eastman Avenue in Green Bay, Wisconsin, at Paper Machine 10F; the Alternative Method of Energy Control procedure did not address all energy sources, such as but not limited to hydraulic and pneumatic energy sources for the rail and program lift J hooks while employees were threading and tying the tail on the core shaft exposing employees to accidental/inadvertent restart of equipment. (b) On or about January 17, 2023, at an establishment located at 501 Eastman Avenue in Green Bay, Wisconsin, at Paper Machine 10F; the Alternative Method of Energy Control procedure did not provide alternative protective measures while employees were threading the tail by the plybonder and slitters. The Pilz PSEN sl-1.0p 2.1 safety gate system was not used in accordance with the manufacturer's instructions to control hazardous energy.

29 CFR 1910.261(b)(1): Lockouts. Devices such as padlocks shall be provided for locking out the source of power at the main disconnect switch. Before any maintenance, inspection, cleaning, adjusting, or servicing of equipment (electrical, mechanical, or other) that requires entrance into or close contact with the machinery or equipment, the main power disconnect switch or valve, or both, controlling its source of power or flow of material, shall be locked out or blocked off with padlock, blank flange, or similar device. (a) On or about December 13, 2022, at an establishment located at 501 Eastman Avenue in Green Bay, Wisconsin, at Paper Machine 10F, lock out devices were not utilized during the tail tying activities to control each of the energy sources, such as but not limited to electrical, pneumatic, and hydraulic for the plybonder, slitter, inject rail, and J hook program lift. (b) On or about January 17, 2023, at an establishment located at 501 Eastman Avenue in Green Bay, Wisconsin, at Paper Machine 10F, lock out devices were not utilized during the tail tying activities to control each of the energy sources, such as but not limited to electrical, pneumatic, and hydraulic for the plybonder, slitter, inject rail, and J hook program lift. One single lockout device was placed on a hinged guard which did not address all energy sources and was used in a manner which was not approved by the manufacturer.
