29 CFR 1926.451(a)(1): Scaffold components were not capable of supporting, without failure, its own weight and at least four times the maximum intended load applied or transmitted to it.  Location: 80 Garfield Street, Cambridge, MA;  On or about 01/17/2023, employees were permitted to use a pump jack scaffold with a 2x6 in. plank and scrap 2x4 in. planks containing knots and other imperfections. These scaffold planks were not graded by a recognized lumber grading association for this use and were not capable of supporting 4 times the maximum intended load.

Location: 80 Garfield Street, Cambridge, MA;  On or about 01/17/2023, wooden planks used as walking/working platforms between both pumpjack scaffolds were not at least 12 inches (30 cm) wide.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  Location: 80 Garfield Street, Cambridge, MA;  On or about 01/17/2023, employees were permitted to work from a work platform greater than 10-feet in length and extended more than 18-inches beyond its support.  Additionally, no guardrails were provided to blocked employee access to the cantilevered end.

29 CFR 1926.1053(b)(4): A ladder was used for purposes other than the purpose for which it was designed.  Location: 80 Garfield Street, Cambridge, MA;  On or about 01/17/2023, employees were allowed to utilize an adjustable extension ladder as a support and connecting member of a makeshift walking/working surface between two pump jack scaffolds which were set up in the front and side of the residence.
