29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of carbon monoxide listed in Table Z-1 in excess of the 8 hour time-weighted average concentration of 50 parts per million (ppm): Beatrice Bakery Co. - 201 So. 5th St. - Beatrice, Nebraska 68310 - On or about January 10, 2023, an employee working in the new warehouse building, was exposed to an 8 hour time-weighted average concentration of 125.2 ppm carbon monoxide for 240 minutes, which exceeds the Permissible Exposure Limit by 250%. Exposure calculations include a zero increment for 240 minutes not included in the carboxyhemoglobin calculations.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: Beatrice Bakery Co. - 201 So. 5th St. - Beatrice, Nebraska 68310 - On or about January 9 and 10, 2023, employee(s) working in the improperly ventilated new warehouse building, were exposed to high levels of carbon monoxide from an LP powered forklift and propane heaters. The employee(s) had not been trained on the hazards, exposure symptoms, and means to protect themselves from carbon monoxide gas produced by the forklift and heaters. Two employees were diagnosed with carbon monoxide poisoning at the local hospital.
