No. 10A-1970: (a) Texas Road House Construction Site, Fort Campbell, KY - center hallway of building interior: On or about 1/18/2023, and at times prior, the employer exposed employees to burns and other fire related injuries while conducting welding operations on pipes at ceiling height while on a Genie GR-20 vertical lift, in that proper inspection and maintenance was not performed and a fully discharged fire extinguisher with the pull pin missing was used for fire prevention.
