29 CFR 1910.215(a)(4): On offhand grinding machines, a work rest was not available to be used to support the work and the work rest available was not adjusted closely to the wheel with a maximum opening of one-eighth inch. The Machine Shop: The wheel to the bench grinder used to fabricate metal was more than one-eight inch from the work rest exposing employees to struck-by and in running nip points hazards.

The Machine Shop: The Bridgeport milling machine used for part repair and fabrication was not equipped with a chuck guard. An employee was exposed to rotating part hazards. b. The Machine Shop: The JET drill press used for part repair and fabrication was not equipped with a chuck guard. An employee was to rotating part hazards.
