(a) On the 7th floor of the Urea Plant, located at 23 Columbia Nitrogen Drive, Augusta, GA: On or about January 1, 2023, employees clearing an obstructed overflow line in the urea plant, were exposed to struck by and thermal burn hazards, in that the employer did not ensure the employees were wearing personal protective equipment.

(a) On the 7th floor of the Urea Plant, located at 23 Columbia Nitrogen Drive, Augusta, GA: On or about January 1, 2023, an employee engaged in clearing an obstructed overflow line in the urea plant, suffered second degree burns on his body, in that the employer did not ensure lockout or tagout devices were applied to equipment such-as but not limited to the condensate hose, the 525 lb. steam line and the obstructed overflow line.
