Location - 14 Cherry Street, Medford, MA;   On or about January 18, 2023, employees were exposed to falls while working/walking from cantilevered scaffold platforms in the following instances:  Instance A) Two employees were working/walking on a pump jack scaffold platform that extended greater than seven feet on left side and six-feet on right side of supports.  Instance B) An employee was working/walking on a ladder jack scaffold platform that extended greater than four feet on left side and three feet on right side of supports.

Location - 14 Cherry Street, Medford, MA. Instance B) Two employees were working/walking on the left rear pump jack scaffold system that had an insufficient mudsill to support scaffold.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Location - 14 Cherry Street, Medford, MA;  On or about January 18, 2023, employees were exposed to fall hazards from incorrect scaffold access in the following instances:  Instance A) Two employees utilized an aluminum extension ladder that was set-up against the work shelf and not the pump jack scaffold platform. The placement of the ladder in this configuration required the employees to crawl over the workbench to access the scaffold platform, further increasing the likelihood of a fall.   Instance B) An employee on the right side of the house utilized the ladder(s) supporting the scaffold plank to access the work area.  Employee were observed climbing over the ladder jacks to then access the scaffold platform.

Instance B) Two employees working/walking at a height greater than 11 feet on the left rear pump jack scaffold were not protected from falling to lower level.  Instance C) An employee working/walking at a height greater than 16 feet on the right side of the house ladder jack scaffold was not protected from falling to lower level.

The lader was set up at a 62� whereas the manufacturer requires 75� angle.
