The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about December 12, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, a Print Screen Operator helped grease the M&R Sportsman Automatic Screen Printing Press #3 inside the safety cable area without having exclusive control of the energy sources that may unexpectedly activate the automatic Screen Printing Machine.   As a result, the remaining applicable energy control elements, involving machine or equipment [1910.147(d)(2)], machine isolation [1910.147(d)(3)], lockout/tagout application [1910.147(d)4)], dissipation of residual energy [1910.147(d)(5)(i)] and verification of isolation [1910.147(d)(6)] were not implemented to protect employees during machine servicing/maintenance activities.

29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  a) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, the ingoing nip point created by the conveyor belt and tail pulley was not guarded on the M&R Sprint 2000 Gas Screen Printing Conveyor Dryer #1 (Model Number SD2K72080112082036A1, Serial Number 877646459S) on the unload conveyor on each side of the conveyor belt.  b) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, the ingoing nip point created by the conveyor belt and tail pulley was not guarded on the M&R Sprint 2000 Gas Screen Printing Conveyor Dryer #2, (Model Number SD2K72080112082036A1, Serial Number 050772055S)  on the unload conveyor closest to the stacking table.  c) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, the shirt feed opening ingoing nip point created by two rollers and the crush point created by the automated shirt table movement towards the shirt feed opening of the M&R Amscomatic K-895 Folding and Bagging Machine, (Model Number K89511160, Serial Number 04103556K) was not guarded.  d) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, the ingoing nip points created by the unload conveyor belt and tail pulley of the M&R Amscomatic K-895 Folding and Bagging Machine, (Model Number K89511160, Serial Number 04103556K) were not guarded.  e) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, the ingoing nip points created by the unload conveyor belt and tail pulley of the Pacemaker Packaging Corp. Pacemaker Model 5'8"14, Serial Number 9849 Bagging Machine were not guarded.  f) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, the Operator and the Unload Employee of the M&R Sportsman Automatic Screen Printing Press #1 were not prevented from walking into the area where the platens automatically rotate to the shirt loading station and the first screen printing station and being struck or caught between the fixed portion of the first screen printing station and the rotating platens.  g) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, the Operator and the Unload Employee of the M&R Diamondback E Automatic Screen Printing Press #2 were not prevented from walking into the area where the platens automatically rotate to the shirt loading station and the first screen printing station and being struck or caught between the fixed portion of the first screen printing station and the rotating platens.  h) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, the Operator of the M&R Sportsman Automatic Screen Printing Press #3 was not prevented from walking into the area where the platens automatically rotate to the shirt loading station and the first screen printing station and being struck or caught between the fixed portion of the first screen printing station and the rotating platens.

Ashland Avenue, Green Bay, WI, a spray bottle labeled DP Magic did not have the associated hazards of the contents on the container in the Screen Printing Department.  d) On or about January 18, 2023, at an establishment located at 2280 S. Ashland Avenue, Green Bay, WI, a spray bottle with an illegible content identity did not have the associated hazards of the contents on the container in the Screen Printing Department.
