29 CFR  1910.253(b)(4)(iii):  Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least:  a) On or about January 19, 2023, in the welding shop, employees were engaged in work activities related to the manufacture of  trailers where oxygen cylinders were stored within 20 feet of propane tanks and acetylene cylinders.

29 CFR  1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:   On or about January 19, 2023, throughout the facility, employees were engaged in work activities related to manufacturing activities were exposed to electrical deficiencies, such as, but not limited to the following:  (a) Flexible cords and cables were used in lieu of permanent wiring where run through holes in electrical panels and switch enclosures that were not protected from damage; (b) Conduit fittings where  conductors enter panels were not secured; (c) Junction box hanging from flexible conduit running over a refrigerator; (d)  An open junction box was mounted to the metal frame between the transformer and the panel that was missing its cover; (e) Live electrical parts over 50-V were not guarded.

29 CFR  1910.22(a)(1):  The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:   On or about January 19, 2023,  in the welding shop, employees were performing work activities related to manufacturing of utility trailers were exposed to the following: (a)  Along the western wall, metal dust and shavings accumulated on the floor and electrical wiring; (b) In the northwestern corner, debris including metal screws accumulated on the floor; (c) On the southern side, near the entrance, buckets of parts and equipment were stored on the floor; (d) On the southern side, near the entrance, material being stored was not orderly.
