29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems. a. On or about 1/19/2023, at 2650 Lakepointe Parkway, Odessa, FL., employees were exposed to trip and fall hazards, in that, the bumper of the delivery trucks were not at the same elevation as the dock, which created an approximate 10" wide gap with a depth of 2.5 inches to 4 inches.
