29 CFR  1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  a) On or about January 19, 2023, on the south side of the residence, where an employee was exposed to a fall hazard greater than 6 feet while applying flashing tape from a pump jack scaffold where the end of the scaffold planking extended greater than 18 inches from the end of the scaffold.

29 CFR  1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high.  a) On or about January 19, 2023, on the south side of the residence, where an employee was exposed to a fall hazard greater than 6 feet while applying flashing tape from a pump jack scaffold that was not equipped with a midrail.
