29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On or about August 20, 2023, at an establishment located at 1564 South Koeller Street in Oshkosh, Wisconsin; the main grill power cord was not provided strain relief. The protective sheath was damaged and pulled back exposing the conductors.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made. On or about August 20, 2023, at an establishment located at 1564 South Koeller Street in Oshkosh, Wisconsin; after a visual inspection of the main grill cord it was identified that the protective sheath was degraded, exposing copper wire of the conductors. The grill was not removed from service until repairs were made to render the equipment safe.
