29 CFR  1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  On or about 1/19/2023 at the Engineering Department of the VA medical facility located at 1201 NW 16th St, Miami, FL, 33125, employees were exposed to a struck by hazard when an unsecured oxygen cylinder, allegedly empty, was not marked (CGA P-1-1965, Section 3.1.11).

On or about 1/19/2023 at the Engineering Department of the VA medical facility located at 1201 N.W 16th St., Miami, FL, 33125. Oxygen and acetylene cylinders were stored together chained to a cart with regulators on, exposing employees to a fire and explosion hazard.
