29 CFR 1926.405 (a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: On or about January 20, 2023, employees engaged in commercial construction activities were exposed to electrical shock hazards while utilizing an extension cord that was being run through a door without protection from accidental damage in the following instances: 1) Orange extension cord running through door to General Contractor's Trailer. 2) Black Power Cord running from generator through front door of building.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about January 20, 2023, employees engaged in commercial roofing activities, near the company trailer there were two extension cords daisy chained together. This condition exposed employees to electrical shock hazards.
