29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes: On or about January 20, 2022, employees engaged in commercial construction operations were exposed to trip and/or fall hazards due to the employer not barricading two holes (5 feet 4 inches long, 1 foot 8 inches wide, 6 inches deep) with rebar cement forms in the following instances: 1) In front of the main entrance walkway. (North of the building). 2) Next to walkways used to the east of the building.
