29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) At the jobsite, one employee was working in the bottom of the excavation conducting work up to approximately 10 feet deep without an adequate protective system in place, exposing the employees to the hazard of a cave-in.
