29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) Northwest corner: On January 20, 2023, and times prior the controlling employer Engineered Structures, Inc (ESI) did not have an effective system in place to identify and correct hazards to which their subcontractors employees were exposed. Hazards on a framed scaffold included having a 21-inch space between the face of the building and the scaffold, not having a safe means of access for the scaffold, and the top level being more the two (2) feet down from an access point. This exposed the sub-contractors' employees to fall hazards up to approximately 48-feet at the top of the penthouse parapet wall to the ground below. Note) Certification and documentation of corrective actions are required for this item.
