29 CFR  1910.102(a):  The employer did not ensure that the in-plant transfer, handling, storage, and use of acetylene in cylinders complied with the provisions of CGA Pamphlet G-1-2009 ("Acetylene"), which is incorporated by reference as specified in 29 CFR 1910.6:  a) On or about January 23rd, 2023 in the DPW Machine Shop, employees were exposed to fire hazards due to large, empty acetylene cylinder tanks being stored in the same vicinity as full tanks, and employer did not ensure that CGA Pamphlet G-1-2009 was followed as it includes in 5.2, "Empty cylinders should be stored separately from full cylinders. Empty cylinders should be marked with the letters MT or EMPTY to avoid confusion;"  b) On or about January 23rd, 2023 in the DPW Machine Shop, employees were exposed to fire hazards and struck by hazards due to acetylene cylinder tanks being stored unsecured, and employer did not ensure that CGA Pamphlet G-1-2009 was followed as it includes in 6.1, "When moving acetylene cylinders, as in storing them, it is important to remember that they should not be subjected to abnormal mechanical shocks that might damage the cylinders, the valve, or the fusible metal pressure relief devices."

29 CFR  1910.212(b): Machine designed for fixed location was not securely anchored to prevent walking or moving:  (a) DPW Machine Shop, center of East wall.  On or about January 23rd the employer exposed employees to crushing injuries, entanglement hazards, and hazards associated with flying debris, in that a Baileigh Industrial 1500VS drill press was not secured to the floor.

29 CFR  1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least 1 hour  (a) DPW Machine Shop, outside the West side of building.  On or about January 23rd, 2023 the employer exposed employee to death and fire related injuries, in that an Oxygen cylinder was stored next to an acetylene cylinder and a welding generator, when not in use, with no fire rated barrier to separate them.

29 CFR  1910.253(b)(5)(ii)(D): Cylinders were not secured on a special truck, regulators were not removed and valve protection caps, when provided for, were not put in place before cylinders were moved.  (a) DPW Machine Shop, outside the West side of building.  On or about January 23rd, 2023 the employer exposed employees to death or injury from fire, explosive, or struck by hazards, in that acetylene and oxygen cylinders were moved from jobsite to jobsite with the regulators still attached and the valve protection caps not installed.

29 CFR  1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI):  (a) DPW Machine shop and other worksite locations on Fort Campbell: On or about 24 January, 2023 and at times prior, the employer exposed employee to lung, sinus, and nasal cancers as well as severe dermatitis, in that the employee's exposure to chromium while  performing welding operations on stainless steel was not monitored to determine exposure levels.

29 CFR  1910.1026(l)(2)(i)(A): The employer did not provide appropriate information and training for all employees exposed to chromium (VI), in that employees could not demonstrate knowledge of the contents of the Chromium (VI) standard, 29 CFR 1910.1026:  (a) DPW Machine Shop: On or about 24 January, 2023 and at times prior, the employer exposed employee to lung, nasal, and sinus caner as well as severe dermatitis, in that the employer did not include Hexavalent Chromium as part of the employees HAZCOM training.
