Plastics Corporation at 3660 Dyess Avenue, Rapid City, SD 57701:  On or about and at times prior to January 23, 2023, the employer did not ensure that electrical equipment was effectively closed, in that an electrical outlet junction box within the production area was missing a knockout at junction box panel LC #27.  This condition exposed employees to shock, electrical, and fire hazards associated with exposed wiring on the electrical boxes.
