29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: 160 Shrewsbury St. Boylston, MA 01505: On or about January 23, 2023, two employees of All Needs Contracting Services, Inc. were seen working in and around an unguarded hole measuring 150 feet long, 80 feet wide, and 24 inches in depth, on the north-west side of the building, while installing concrete forms.
