At a new residential construction site, located at, or near, 40 Rum Runner Way, Fruit Cove Jacksonville, Florida: On or about January 24, 2023, the employer exposed employees to struck-by flying projectile hazards, in that eye protection in compliance with ANSI/ISEA Z87.1-2010 was not utilized by employees while operating a hand-held circular saw.

At a new residential construction site, located at, or near, 40 Rum Runner Way, Fruit Cove Jacksonville, Florida: On or about January 24, 2023, the employer exposed an employee to a fall hazard, ranging from 6 to 10-foot, in that a portable extension ladder was not extending at least 3 feet above the upper landing surface, as employee climbed and descended the ladder.

At the east side of a new residential construction site, located at, or near, 40 Rum Runner Way, Fruit Cove Jacksonville, Florida: On or about January 24, 2023, the employer exposed an employee to fall hazards, ranging from 6 to 10-foot, in that a portable A-Fram type ladder was not placed on a stable and level surface as an employee used the ladder to install exterior siding to a residence.
