29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. A.) On or about January 18, 2023, while performing demolition operations on a low slope roof, employees were exposed to fall hazards associated with skylights without covers or guards placed on or around them, exposing employees to fall hazards of 18' to the concrete floor below.

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. A.) On or about January 18, 2023 and prior to that date, employees were not trained in the recognition and avoidance of fall hazards in that they were working on the roof of a commercial structure while not utilizing adequate fall protections systems in that they were exposed to falling through unguarded skylights.
