(a) Building 6695, North wall, left of rear exit door, employer exposed employees to death and fire related injuries, in that the pathway to the rear exit door was reduced to a width of 15 inches which is less than the required 28 inch minimum for a building with at least two exits.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casings enclosing shafting completely: (a): Vicinity of DPW Landfill on Fort Campbell, KY: On or about 1/24/2023, and at times prior, employer exposed employees to caught in and amputation hazard, in that , the rotating shaft casing connecting a Kubota M6 tractor and a pull behind rotary cutter, was not protected with a PTO cover. (b): Vicinity of DPW Landfill on Fort Campbell, KY: On or about 1/24/2023, and at times prior, employer exposed employees to caught in and amputation hazard, in that , the rotating shaft casing connecting the center shaft of pull behind rotary cutter to the left cutting wing, was not protected with a PTO cover.
