29 CFR  1910.67(c)(2)(v):A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  Richard's Tree & Crane Service, LLC on or about January 24, 2023, at the worksite located at 32163 Ventanas Cr. In Avon Lake, OH: The employer failed to ensure that a personal fall arrest system was used by employee(s) who performed tree trimming operations from an Easy-Lift (model RA26; s/n 2379) aerial lift.  An employee performing work in this condition was exposed to an approximate 30-50 ft. fall to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
