29 CFR  1910.333(a)(1): Live parts to which an employee would be exposed were not deenergized before the employee worked on or near them, and the employer did not demonstrate that deenergizing introduced additional or increased hazards or was infeasible due to equipment design or operational limitations:  a.) On or about January 23, 2023 at Costech Labs, employees were exposed to the hazards of electrical shock and arc flash while installing approximately 50 feet of EMT conduit and wiring to connect a piece of equipment to a 200 amp breaker in a 480 V, 800 amp switchgear without de-energizing the switchgear.

29 CFR  1910.335(a)(2)(i): When working near exposed energized conductors or circuit parts, each employee did not use insulated tools or handling equipment when the tools or handling equipment might have made contact with such conductors or parts:  a) On or about January 23, 2023 at Costech Labs, two employees were exposed to the hazards of electrical shock and arc flash while installing approximately 50 feet of EMT conduit and wiring to connect a piece of equipment to a 200 amp breaker in a 480 V, 800 amp switchgear without de-energizing the switchgear and while using uninsulated tools such as pliers.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.  This documentation may include but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
