29 CFR  1910.36(a)(2): An exit must be separated by fire resistant materials. Construction materials used to separate an exit from other parts of the workplace must have a one-hour fire resistance-rating if the exit connects three or fewer stories and a two-hour fire resistance-rating if the exit connects four or more stories.  (a)Bldg. 3902, employee breakroom/office, South wall: On or about January 25th 2023 the employer exposed employees to death or serious injury from fire hazards in that an exit door required to have a fire rating was rendered ineffective by the removal of the door knob leaving an open hole.

29 CFR  1910.102(a):  The employer did not ensure that the in-plant transfer, handling, storage, and use of acetylene in cylinders complied with the provisions of CGA Pamphlet G-1-2009 ("Acetylene"), which is incorporated by reference as specified in 29 CFR 1910.6:  (a) On or about January 25th, 2023 in Bldg. 3902, center of main floor, West side of chillers: employees were exposed to fire hazards and struck by hazards due to acetylene cylinder tanks being stored unsecured, and employer did not ensure that CGA Pamphlet G-1-2009 was followed as it includes in 6.1, "When moving acetylene cylinders, as in storing them, it is important to remember that they should not be subjected to abnormal mechanical shocks that might damage the cylinders, the valve, or the fusible metal pressure relief devices."
