29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval. Location: Basement of 60 Kilmarnock St. Boston, MA On or about January 24th, 2023, an employee was exposed to a vehicle tipping hazard, where a powered industrial truck was utilizing an unauthorized attachment.
