On or about January 24, 2023, and at times prior thereto, maintenance personnel authorized to perform repairs and install replacement parts on facility machinery had not received required training for authorized personnel, exposing employees to the hazard of being caught-in the machine and moving parts during unexpected startup.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about January 24, 2023, and at times prior thereto, two employees use a ladder to reach over the interlock doors of a blow molding machine to adjust extrusion bolts during operation, exposing the employees to the hazard of being caught-in the moving machine parts during its operating cycle.
